Motor Accidents
To intimate your claim, you must complete the supporting documentation listed below. To Police Report.
- Vehicle Registration Card (Dafter) copy from both sides Driving License copy from both sides.
- A filled Claim Form signed by Insured Policy Excess deposit Slip.
- Vehicle Photos and Policy Copy.

Medical Claims
- A completed and signed claim form with all the necessary facts
- A copy of the patient’s medical card
- The Intimation/pre-approval copy of the mail- Intimated or received
- All the original bills/invoices and their receipts towards payments made
- Medical records/medical reports stating the diagnosis & treatment details
- A prescription from the doctor if any medicine bills are submitted
- Investigation reports if any investigation bills are submitted Break up towards the submitted bills/Invoice if any
Medical
Download the Claim Reimbursement Form, and prepare the documents listed on the Reimbursement Checklist, to submit a request for a medical claim reimbursement. Within 45 days following treatment, submit all necessary documentation.

Personal Accident Insurance
Download the claim Form from the list below. Please complete the form and submit it to our head office together with the original insurance document and any necessary collaterals to substantiate your claim.

Travel
If a non-emergency incident occurs, you must submit a claim form as soon as possible. Non-emergency claims will need to be submitted within 4 months after getting back to your home country.
Additionally, the completed claim form should be sent to Swan International Assistance with the original invoices, proof of ownership, travel papers, and any other pertinent information along with the following:
Loss of Baggage
- A copy of check reimbursed from the airline
- A letter from the airline confirming the loss
Flight cancelation
- Proof of the cancellation: boarding pass, letter from the airline, flight history & original receipts/invoices for the expenses being claimed
Trip cancelation & curtailment
- The original receipts/invoices for expenses being claimed
- Hospital/doctor reports/records
- If the expenses are a result of an incident, a copy of the police report
- Death certificate
Delay of Baggage
- Proof of the delay from the airline
- Flight history
- A letter or any other forms of proof showing the date of reception
- Receipts of all the first necessity items purchased
